To better care for our patients, we have partnered with CHADIS.

CHADIS is a secure on line system that delivers questionnaires that help your pediatrician review the health and development of your child.   The process also allows you to share your questions and concerns with the pediatrician before the visit.

MemoryBook Care Portal is a feature of CHADIS which allows parents to capture their child’s special memories and achievements in a keepsake format. It is also a way for your pediatrician to share resources such as community programs, books, and health information specific to your child’s development and health.

For first time users, Magnolia Pediatrics will send you an Email with an invitation code. Follow the step to register. You can also register on your own at www.chadis.com, using our office phone number as your invitation code, 4153808700.

For registered users, you will be sent age and visit specific questionnaire reminders, by email, one week prior to child’s well check appointment. Questionnaires will take about 10-20 minutes to fill out and can be done at the convenience of your home. If you forget to fill out the questionnaire before the visit, please come 20 minutes prior to your appointment time to fill out the questionnaire at the office.
